INFORMATION ON THE PROCESSING OF PERSONAL DATA COLLECTED THROUGH THE KTW.LEGAL SERVICE CONTACT FORM

In relation to the provisions of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter referred to as GDPR), we would like to inform you as a person using the contact form on the KTW.LEGAL website that:

I. THE IDENTITY AND CONTACT DETAILS OF THE ADMINISTRATOR

The administrator of your personal data (hereinafter referred to as the “Administrator”) is the company KTW.LEGAL PŁONKA MEDAJ SOBOTA RADCOWIE PRAWNI SP.P. in Katowice (40- 53), ul. II. THE PURPOSE AND LEGAL FRAMEWORK OF PROCESSING PERSONAL DATA

Your personal data will be processed in order to answer the inquiry submitted via the contact form, pursuant to GDPR Art. 6.1.a , i.e. based on the consent you granted.

III. INFORMATION ON THE RECIPIENTS OF PERSONAL DATA

The recipients of your personal data are:
1. the Administrator and the Administrator’s representatives authorized to process personal data
2. the hosting provider of the server on which the website of the Administrator’s service is posted (“AGNAT” Sp. z oo, address: Bydgoszcz, 85 – 022, Gdańska 119)

IV. TRANSMISSION OF PERSONAL DATA TO A THIRD COUNTRY

The Administrator does not transfer your personal data to a third country.

V. PERSONAL DATA RETENTION PERIOD

Your personal data will be stored by the Administrator for a period not longer than 3 years from the date of their collection, until the end of the calendar year, and in the event that other applicable laws require the storage of specific data for a longer period of time, such data will be stored until such longer time expires.

VI. RIGHTS OF THE DATA SUBJECT

You have the right to request the Administrator to grant you access to your personal data, correct it, delete it (in accordance with GDPR Art. 17) or limit its processing (in accordance with GDPR Art. 18.1). You have the right to transfer your personal data(in accordance with GDPR Art 20.1) and the right to withdraw consent at any time, without affecting the lawfulness of processing based on consent before its withdrawal. In order to exercise any of the above rights, a relevant request should be sent to the administrator, including in person, by phone, e-mail or by mail, using the administrator’s contact details indicated in point I. above.

VII. THE RIGHT TO LODGE A COMPLAINT WITH A SUPERVISORY AUTHORITY

You have the right to lodge a complaint with the supervisory authority if you believe that the processing of your personal data by the Administrator violates the provisions of the GDPR. Pursuant to GDPR Art. 77, the data subject has the right to lodge a complaint with a supervisory authority, in particular in the Member State of their habitual residence, their place of work or the place of an alleged violation of the GDPR. The supervisory authority in the Republic of Poland is the President of the Personal Data Protection Office.

VIII. THE REQUIREMENT TO PROVIDE PERSONAL DATA

Providing your personal data for the purpose of the contract is voluntary, but indispensable to answer the inquiry submitted via the contact form. Your refusal to provide personal data will result in our inability to answer your inquiry.

IX. AUTOMATED DECISION-MAKING, INCLUDING PROFILING

The administrator does not perform any automated decision-making nor profiling.”
